Request Service Disconnect

If you are moving or no longer require District water services, please complete this form to have service disconnected. It is your responsibility to complete this form to have service disconnected; charges will continue to accrue in the name of the account holder until this form has been completed and processed. Most disconnect services can be completed within one business day, but it is advised to call as far in advance as possible to assure a specific disconnect date. 

Date that you would like service disconnected. Service disconnect requests made after 12 PM EST will be done the following business day (Monday - Friday).
Service disconnect must be requested by the account holder.
For security purposes, a valid driver's license number is required to process your request. If you do not have this information please call our office for further instructions.
